What is an Assessment?

The Assessment element allows you to ask multiple choice questions as part of a form. 

For example, if you are building a Workforce Compliance Requirement form, you could build an assessment to test a user's knowledge after attending a training session.

How do I set up an Assessment?

From within the Form Editor, drag the Assessment option from the left hand elements pane, into the middle canvas: 


When you first drag the element, it will turn red, as can be seen in the image below:  


First, set the pass mark for the assessment; this is the number of questions that must be answered correctly in order for the assessment to be passed (you can edit this at any time should you wish to come back to this at the end). This can be entered either in the middle canvas, or on the left hand panel 

Note, you must ensure that your pass mark matches the number of marks available based on the questions asked. Please refer to the section later in this guide called "Setting question marks" for clarification:




Next we need to add our first bank of questions. Question banks allow for the randomisation of questions being shown. For example, you may have two topics of questions, and want to ensure that an equal number of randomised questions are pulled covering both topics. You would achieve this by splitting the two topics into two separate banks. 


Click Add Question bank. Your first question bank will appear as per the below:


Click on the newly added question bank to reveal further options. Enter your first question into the Question field




Click Add Question. Your question will now appear below the Question bank.



Click on your question in the question bank to reveal the options to add the question answers.

Type your first answer and click Add Option



Note - when building your questions, it is important to be aware that all questions are single selection multiple choice (i.e. the user will only be able to select a single answer, they will not be able to select two or more answers)


Continue adding your answer options and click Add Option after each. 


Once all your answer options have been added you need to set which of the options is the correct answer. 

To do this, click on the correct answer.

From either the middle canvas, or the left hand panel, set the Is answer field to Yes.


You have now successfully added your first question. A preview of how it will look can be seen on the right hand preview panel:


To add your next question, from within the Question Bank, type your next question into the Question... field and click Add Question:


Continue the same process as before, adding your question answer options and selecting which of these is correct.


FORM EDITOR TIP

When you are building your assessment, your canvas could start to get quite busy due to the cascading nature of the elements. 

You can  minimise elements down at any time by clicking on the two small arrows on the right hand side of each element/field in the canvas

Here you can see the same form, but with each of the questions minimised using the arrows shown above - it's much cleaner and easier to read!


Setting Question Marks

Once you have created your question bank, you need to make sure that your pass mark matches the question marks available. 


Each question as you have built it will have been given 1 mark by default. You are able to change this, which means you can weight questions to make some more important than others, should you need to do so. 


You must ensure that the total pass mark you set at the beginning matches the number of marks available. E.g. If you set a pass mark of 10 but only make 5 marks available, your users cannot pass the assessment!


You can view the pass mark at the top of the assessment.

You can view the marks assigned for each question against each question in the Question bank. You can edit both of these accordingly. 



Shuffling the Question Order

For some assessments, you may wish questions to appear in a random order, rather than always in the sequence that you have created them.


To set this, on the Question bank, set the Shuffle order to Yes




Select Questions at Random

As well as shuffling the order the questions appear in, you can also choose to only pull a certain number of questions through into the assessment. 


To set this, on the Question bank, select the number of questions you would like to pull through from the Select at random... drop down



Beware when using this option, if you have weighted some of your questions as Radar Healthcare will not take the available marks into account when randomly selecting the questions. We recommend only using this option if all your questions are equally weighted.
